Home Health RN

We are seeking a dedicated Home Health Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the home environment to determine specific home health care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change. 

Salary determined on experience and licensure and if full-time or PRN.

Qualifications:
Must possess current Illinois RN licensure
Home Health experience preferred but not required
Current CPR
Valid IL driver’s license and auto insurance policy

Licensed Practical Nurse/LPN

Licensed Practical/Vocational Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies.

Home Health Aide

A person who is specifically trained, competent and performs assigned functions of personal care to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN). Have a sympathetic attitude toward the care of the sick and elderly. Ability to carry out directions, read and write. Maturity and ability to deal effectively with the demands of the job.

Qualifications:
Must meet Medicare Conditions of Participation for Home Health Aide training program and competency
Registered with IDPH Aide Registry
Current CPR card
Current Physical TB Test

Physical Therapist

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Physical Therapy Assistant

As a Physical Therapy Assistant (LPTA) administer physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Physical Therapist (PT).

Current Illinois license as a Physical Therapy Assistant

Occupational Therapist

The Occupational Therapist is responsible for providing occupational therapy services to patients including but will not be limited to; assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care. The Occupational Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for Occupational therapy.

Home Care Home Base experience preferred but not required

Occupational Therapy Assistant

As a Certified Occupational Therapy Assistant (COTA) administers occupational therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Occupational Therapist and Director of Clinical Services/Clinical Manager.

Speech Therapist

As a Speech Language Pathologist (SLP) administers speech therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager. Speech therapy services are furnished only by or under the supervision of a qualified speech pathologist or audiologist.

Medical Social Worker

As a Medical Social Worker (MSW) provide social work services to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager. Services are furnished by a qualified social worker or by a qualified social work assistant under the supervision of a qualified social worker.
